Registered Nurse
Support Health, Build Momentum
At The Momentum Project, our nurses play a critical role in promoting the health, safety, and well-being of individuals in our Shared Living program. We are looking for compassionate and detail-oriented nurses who are passionate about helping children and adults with developmental disabilities live healthier, more independent lives in family-style homes.
What You’ll Do

Complete monthly face-to-face health visits with individuals in Shared Living

Perform nursing assessments and complete monthly nursing summaries

Monitor health status, medication management, and physician orders

Train Host Home providers on new medications, medical equipment, and care procedures

Complete Level I Medication Aide (L1MA) training and refresher courses with Host Home providers and staff

Complete follow-up visits after hospital discharges or health events

Document services accurately and timely in compliance with state and agency requirements

Collaborate with behavior analysts, case managers, families, and Host Home providers

Participate in placement discussions, health planning, and ongoing care reviews
What We Are Looking For
-
Active Registered Nurse (RN) license in Missouri
-
One year experience working with Department of Mental Health individuals (preferred, not required)
-
Experience supporting individuals with developmental disabilities (preferred, not required)
-
Strong documentation, communication, and teaching skills
-
Experience providing caregiver and level one mediation aide training (preferred, not required)
-
Reliable transportation for travel within the St. Louis metro area
-
Ability to work independently while maintaining strong communication with the team
