Host Home Manager

At The Momentum Project, Host Home Managers play a vital role in supporting the success and stability of Shared Living arrangements. Our Host Home Managers work directly with Host Home providers and individuals to ensure that homes are safe, supportive, and focused on helping individuals build meaningful lives in their communities. If you are passionate about working with families, promoting independence, and creating opportunities for growth, we invite you to join our team.

What You'll Do

  • Conduct home visits with Host Home providers and individuals at least monthly

  • Complete Host Home documentation including monthly summaries, training records, and home visit reports

  • Provide ongoing support, training, and guidance to Host Home providers

  • Monitor individual health, safety, rights, and overall well-being

  • Assist in placement planning, matching individuals with appropriate Host Home providers

  • Respond to critical incidents, participate in investigations when needed, and follow up on concerns

  • Collaborate with an interdisciplinary support team

  • Ensure Host Homes meet agency policies, DMH regulations, and licensing requirements

  • Support individuals in working toward greater independence, community inclusion, and personal goals

What We Are Looking For

  • Bachelor’s degree in human services, social work, psychology, or a related field (preferred)

  • At least two years of experience supporting individuals with developmental disabilities

  • Strong documentation, communication, and organizational skills

  • Ability to travel within the St. Louis metro area

  • Knowledge of DMH Shared Living regulations (preferred but not required — training provided)

  • Commitment to protecting individual rights and promoting quality of life

  • Ability to work independently while collaborating closely with support teams
