What is a Host Home?
A Host Home is a family-style living arrangement where an individual with a developmental disability lives in the home of a trained caregiver. The Host Home provider offers daily support, helps build independent living skills, and encourages participation in family and community activities. Host Homes provide more than care — they create a sense of belonging and stability that helps individuals reach their goals
Who Can be a Host Home Provider?
Host Home providers come from all walks of life. You can be single, married, have children, or live alone. What matters most is your ability and desire to provide a safe, stable, and welcoming home.
To be eligible, providers must:

Have a passion for wanting to make a difference

Be at least 18 years old

Pass background checks and screenings

Complete required Host Home training
-
Have a stable home environment (own or rent)
-
Be able to transport the individual to appointments and activities
-
Be comfortable and able to work as a independent contractor.
What Are the Responsibilities?
As a Host Home provider, you support an individual’s daily life and independence while helping them stay connected to their community. Responsibilities include:

Providing assistance with daily living activities

Supporting personal goals and skill development

Administering medications as needed
-
Encouraging community participation and social connections
-
Creating a safe, nurturing environment where growth is possible
All providers receive full training before placement and ongoing support after an individual moves into the home.

Who is Eligible for Host Home Placement
Host Home placements through The Momentum Project are available to adults with developmental disabilities who are approved for services through Missouri’s Medicaid waiver programs, as well as to children and youth in foster care with developmental disabilities who require more support than a traditional foster home can provide. Shared Living is designed for individuals who would benefit from living in a stable, family-style environment where daily support, skill-building, community involvement, and long-term personal growth are the focus.
